
Grant requirements
Please be mindful of our requirements listed below before sending a letter of intent or applying for a grant.
- You must clearly demonstrate that the organization is within a 15 to 20-mile radius of Highland Park Hospital (Rt. 120 to the North, Rt. 45 to the West, and Rt. 58 to the South). Please refer to the map which delineates the immediate Highland Park Hospital service area– Zone 1, and the adjacent communities in Zone 2. The population served must reside within those boundaries and directly benefit from the program. Preferential consideration will be given to Zone 1.
- The organization’s principal office is preferred to be within the defined zones. If it is not, you will need to demonstrate that the program takes place in the defined service area and that the individuals who benefit from it live within that defined area as well.
- Please be specific about the population served by demonstrating in numbers and by percentage, where the recipients of the services reside and how much of the program’s participants they represent.
- We will require metrics that reflect the success rate of the program and of the impact on the participants, specifically those individuals in the targeted service area.
- We are interested in the sustainability of your program. Please enumerate your other funding sources.
The Foundation will not consider the following types of grant requests:
- Endowments
- Fundraising events including sponsorships
- Debt reduction
- Direct grants to individuals
- Grants used as a grant to another organization
- Programs outside the service area of Highland Park Hospital as defined above
- Capital projects
- Equipment purchases
- Research

Reapplying?
Please note that if you have received a grant in the past, you only need to submit a Grant Application for the current year after you receive an RFP. A Letter Of Intent is not required if you are invited to apply through an RFP.
